School Based Services 

Counseling Services

Mental health and other support services are provided in a special day class setting for emotionally disturbed children as well as Inclusion Services to children who struggle with emotional issues in their home school. The program also works with children to transition them from higher levels of care back to their home school and prevent children from needing higher levels of care. School based counseling students are referred through the IEP process.

Head Start Consultation Program

Sierra Vista provides both Regional and Migrant Head Start mental health consultation in Merced County. The program provides classroom observations and reports, individual child observations, school teacher consultation, staff training and support, social skills development and parent support. The program increases social role functioning while providing family and teacher support.

Early Mental Health Services

About the Program

Through a grant from First 5 Merced County, Sierra Vista Child & Family Services provides an Early Mental Health Support Program for children from birth through age five and their families. The program provides a continuum of community based mental health services for children and their families including outreach and engagement, individual assessment and service planning, counseling and intervention, parent education and support, group support services for children, and case management and referral. The project also provides mental
health consultation services for preschools and day care providers.
 

Referral, Assessment and Planning

Referrals are accepted from day cares, preschools, Head Start Programs, County and Community Based Agencies, and other service providers. Self-referrals are also encouraged and welcomed. Reasons for referrals include social-emotional delays, behavioral problems, family dysfunction or stress, high risk behaviors, or any other indicator of emerging mental health issues. Referrals will be accepted for children and families that reside in Merced County and have children ages 0-5. Assessments include basic health screening, an assessment of family functioning, parent and child relationships, substance abuse, and family violence screening. A service plan for the family is developed based on child and family needs.

Foster Family & Adoption Services

The Foster Family Program matches children requiring supported specialized foster care with loving, stable foster families. Highly trained and certified foster families provide specialized care to children who are experiencing emotional and behavioral difficulties. Our adoption program provides permanency placement for cihldren who are available for adoption. Services include recruitment, evaluation and approval of adoptive homes, pre-adoptive placement, and post-adoption services and support.
